Cardiovascular Surgery Department

  Cardiovascular Surgery Department of Tianjin Chest Hospital, founded in 1963, is the largest cardiovascular surgery center in Tianjin area.At present, the department consists of three general wards and oneintensive care unit (ICU), with 166 beds.Thediagnosis and treatment scope covers all kinds of cardiovascular surgery. It has been developed as a center of heart and vascular surgery, involving cardiovascular surgery, anesthesiology, cardiopulmonary bypass, ultrasound, imaging and other disciplines, forming a new discipline development model: integrating "clinical front-line, auxiliary departments and basic scientific research ".

  This discipline was awarded as the key clinical discipline of Tianjin in 2018. Currently, it has completed more than 3000 cardiac surgeries annually, ranking the first in Tianjin and the top 10 in China.The volume of off-pump coronary artery bypass surgery has been ranking first in Tianjin and second in China for consecutive three years.At present, it is the affiliated unit of the Quality Control Center of Cardiothoracic Surgery of Tianjin Municipal Health Commission, the national pharmacological clinical trial institution of cardiothoracic vascular Surgery, and is selected by the National Development and Reform Commission to improve the ability of diagnosis and treatment of intractable diseases project reserve, as one of the hundred in China and one of the two in Tianjin.

  In the past three years, the discipline had held more than 20 national education and training projects, and undertaken 15 national, provincial and bureau level fund support projects, with a total of more than 10 million yuan of project funds. It had won 1 second prize and 3 third prize of Tianjin Scientific and Technological Progress and more than 10 items of Tianjin scientific and technological achievements.

This discipline was grouped according to subspecialties, as an important development direction for clinical and scientific research:

(1) Surgical treatment of coronary heart disease

The operation was mainly based on the off-pump coronary artery bypass technique, and the success rate of the operation was more than 99%, and it was awarded the third prize of Tianjin Science and Technology Progress.In 2009, this discipline took the lead in conducting endoscopic great saphenous vein bridge in China, and is now the national training base of this project.In recent years, coronary artery hybridization surgery and left intercostal small incision multi-branch coronary artery bypass grafting have been greatly developed,which further reduced the surgical trauma, resulting as a better long-term prognosis.

(2) Surgical treatment of heart valvular disease

The proportion of biological valve implantation in the department is more than 70%, and the proportion of valvuloplasty surgery is more than 50%, which significantly improves the long-term prognosis of patients.The sinus conversion rate of bipolar radiofrequency ablation and left atrial appendage forceps in the treatment of atrial fibrillation reached the advanced level in China.This discipline was the first to complete total endoscopic minimally invasive valve surgery in Tianjin, and developed technical specifications and actively promoted them.The interventional valve multidisciplinary surgical team completed the first transcatheter aortic valve replacement and transcatheter mitral valve replacement in Tianjin, and won the second prize of Tianjin Scientific and Technological Progress award in 2020.

(3) Surgical treatment of aortic disease

The department treated more than 80% of the patients with aortic dissection in the city. At present, the annual operation volume of Type A dissection is more than 150 cases, and the success rate of operation is more than 90%.More than 200 cases of B-type dissection were performed annually, and the success rate of operation was more than 99%. The team has cooperated with local universities to develop remote referral platform and artificial intelligence follow-up platform to further improve referral efficiency and long-term prognosis of patients.

(4) Surgical treatment of congenital heart disease

This discipline has obvious advantages in the treatment of young and low weight patients with complex congenital heart disease. It routinely performs such difficult surgical procedures as neonatal congenital heart disease surgery, Switch surgery, Rastelli surgery, modified Fontan surgery and so on.The team has been cooperated with German interventional experts to carry out joint diagnosis and treatment projects for years.We established the charity project of “Hui-xin Program”, which reduced the operation cost for more than 1,000 children.

(5) Treatment of critically ill patients and end-stage heart disease

We have rich experience in the application of ECMO in treating severe myocarditis, cardiogenic shock after acute myocardial infarction, low cardiac output after cardiac surgery, and have completed the largest number of cases of severe myocarditis treated by ECMO in China.The first heart transplant was completed in 2021, with good follow-up results.

(6) Comprehensive treatment of cardiac rehabilitation

We are the first center to carry out cardiac rehabilitation for coronary heart disease in China, which had been selected as one of the 11 national certified cardiac rehabilitation centers last year. The length of hospital stay and readmission rate were significantly reduced, after integrated management ofindividualized sports training, patient education, risk factors control and other multidisciplinary treatment.
